A proximal lesions manifests with the Hand of Benediction sign whereas a distal lesion does not. The Hand of Benediction is an active sign most pronounced when the patient is asked to make a fist and the middle and index finger are unable to flex.

Claw Hand. Due to ulnar nerve damage causing paralysis of the medial two lumbricals. It is characterised by unopposed extension at the MCPs and unopposed flexion at the interphalangeal joints of the 4th and 5th digits. The sign is most pronounced when the patient is asked to extend all fingers. It is not to be mistaken with the Hand of Benediction, which affects the 2nd and 3rd digits when making a fist.
